The clinical evidence for most chronic care interventions is sound. What is less well understood — and far more consequential — is why patients stop following them. Not within days, but across months and years.

The answer lies in human behaviour — how motivation fluctuates with context, and how the relationship between knowing what is good for you and actually doing it is far weaker than most care models assume.
